Implementing a five-year strategic plan is about more than achieving short-term goals, it’s about building a foundation that lasts. Preparing for sustainability ensures that programs, initiatives, and supports can continue to grow and thrive well beyond the life of the plan.
By thinking ahead, we can strengthen organizational capacity, secure long-term resources, and cultivate leadership within the community. This forward-looking approach allows us to adapt to changing needs, maintain momentum, and continue advancing equity and well-being for Black families in Frederick County for years to come.

Maternal health is just the beginning of our work to advance equity in Frederick County. The framework we’ve built that is centered on community leadership and lived experience expertise and offers a model for addressing other disparities in the Black community.
We see opportunities to leverage this approach across health, social services, and other systems, collaborating with residents and local organizations to identify needs, co-create solutions, and shift power to those most impacted. By expanding what we’ve learned from maternal health, we aim to tackle inequities broadly and create a thriving, equitable community for all Black residents.
